ClearChoice Home Health is a Minnesota Department of Human Services licensed 245D provider. We serve individuals with disabilities, mental health needs, and complex medical conditions across the Twin Cities. Every person deserves dignified, person-centered care.
Authorized under Minnesota Statute 256B as a 245D and intensive HCBS services provider.
Every team member completes DHS background checks and is listed on the required registry.
We respond to families, referrals, and case managers within one business day, without exception.
